29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a) Production Area- The employer failed to ensure that all areas where hazards created by the point of operation were guarded on machinery, such as the Guifil PE15-34 Press Brake, Model #17900245, and the Guifil PE20-63 Press Brake, Model #014419, to prevent injury to employees during the machine's operation.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1904.32(b)(6): The Summary of Work-Related Injuries and Illnesses (OSHA Form 300A or equivalent) for the previous year was not posted between February 1st and April 30th:    a) The employer failed to post the calendar year 2014 OSHA 300A summary based on the information from the 2014 OSHA 300 log from February 1, 2015 to the date of the inspection, March 4, 2015. 29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    a) Production Area- The employer failed to ensure that electrical equipment, such as the Tripp Lite Waber  Series 6-Outlet Power Strip relocatable power tap that was supplying power to various equipment on the PE1535 Press Brake, was being used only to power audiovisual equipment as permitted under its listing and labeling.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
