29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial trucks were performed by the employer without the manufacturer's prior written approval. Capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly: (a) Shop Area: On or about 2/24/15 the Komatsu powered industrial truck was equipped with a job made lifting device which employees were using to lift steel and it had not been approved by the manufacturer nor were the appropriate plates changed accordingly.
