29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above the floor or platform were not enclosed:  Shear Department - Employees engaged in shearing activities were exposed to caught-between hazards in that the chain and sprocket of the conveyor system next to the Shear machine operations, approximately 22 inches above the floor, was not guarded.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment:  A written certification of the workplace hazard assessment for personal protective equipment had not been developed while requiring employees to wear earplug, gloves, safety shoes, and safety glasses.
