29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes:  On or about February 24, 2015, at the above addressed jobsite, employees were cutting and capping a pressurized natural gas line, without the use of adequate Personal Protective Equipment (gloves/fire resistant clothing and metatarsal guards), therefore exposing the employee to thermal burns and explosive/blast injuries to their eyes/face, head, feet, extremities and hands.

29 CFR 1910.147(a)(2)(iii)(B)(3): documented procedures were not established or followed, and special equipment was not used, which would have provided proven effective protection for employees:  On or about February 24, 2015, at the above addressed jobsite, the employer did not document procedures for employees who were exposed to thermal burns and explosive/blast type injuries while performing cutting and capping operations of a pressurized natural gas line.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  On or about February 24, 2015, at the above addressed jobsite, employees were cutting and capping a pressurized natural gas line, during which the employer had not developed and implemented a Lock Out Tag Out (LOTO) Program, nor provided employee training, therefore exposing these employees to potentially hazardous energy, such as but not limited to; thermal burns and explosive/blast type injuries.
