On 11/17/14 an employee sustained a right knee sprain and was placed on restricted duty for 14 days. b) Progressive Machine & Design, LLC, on or about 3/5/15: The employer did not record the following workplace injuries that met the general recording criteria during calendar year 2015: 1. On 1/5/15 an employee injured their right wrist that resulted in medical treatment, i.e. brace. 2. On 2/12/15 an employee sustained a low back strain that resulted in lost 3 days away from work.
