29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter:    a)  Progressive Machine & Design, LLC, on or about 3/5/15:  The employer did not provide fire extinguisher use training to employees upon initial employment and annually thereafter.         * ABATEMENT NOTE:  By this date the employer must either correct the alleged violation or implement a Fire Safety Policy; as outlined in 29 CFR 1910.38(a) and .39(a) which includes the evacuation requirements of 29 CFR 1910.157(b).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a)  Machine Assembly Tool Room, on or about 3/5/15:  Bridgeport vertical milling machine was not provided with a guard to cover the rotating chuck and tool.    b)  Machine Assembly Tool Room, on or about 3/5/15:  Chevalier vertical milling machine was not provided with a guard to cover the rotating chuck and tool.    c)  Machine Assembly Tool Room, on or about 3/5/15:  Cumins drill press was not provided with a guard to cover the rotating chuck and tool.    d)  Boring Mill Area, on or about 3/5/15:  Two (2) DeVLieg horizontal milling machines were not provided with a guard to cover the rotating chuck and tool.    e) Fabrication Tool Room, on or about 3/5/15:  Three (3) Bridgeport milling machines were used without a guard to cover the rotating chuck and tool.    f)  Fabrication Tool Room, on or about 3/5/15:  Hwacheon lathe was used without a guard to cover the rotating chuck.    g)  Water Jet Area, on or about 3/5/15:  Ridgid drill press was used without a guard to cover the rotating chuck and tool.

29 CFR 1904.7(a):  The employer did not record workplace injuries and illnesses that would be considered to meet the general recording criteria, and therefore to be recordable, if it results in any of the following: death, days away from work, restricted work or transfer to another job, medical treatment beyond first aid, or loss of consciousness.   a)  Progressive Machine & Design, LLC, on or about 3/5/15:   The employer did not record the following workplace injuries that met the general recording criteria during calendar year 2014:      1.    On 11/18/14 an employee sustained a left elbow laceration that resulted in sutures.      2.    On 11/17/14 an employee sustained a right knee sprain and was placed on restricted duty for 14 days.  b)  Progressive Machine & Design, LLC, on or about 3/5/15:  The employer did not record the following workplace injuries that met the general recording criteria during calendar year 2015:      1.    On 1/5/15 an employee injured their right wrist that resulted in medical treatment, i.e. brace.      2.    On 2/12/15 an employee sustained a low back strain that resulted in lost 3 days away from work.
