29 CFR 1910.23(c)(1):     Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.      Worksite located at 107 Railroad Ave, Morrill, NE; Upper level of the mill/storage area:  The employer failed to ensure that a fall hazard located at the edge of the primary walkway leading to two distribution heads was protected with a guardrail or equivalent means.  The employees in the area are exposed to falls of approximately 10 feet.          Abatement certification is required for this violation.  The abatement certification sheet is enclosed with the citations.

29 CFR 1910.178(a)(6):     The employer did not ensure that all nameplates or markings were not maintained in a legible condition:    Worksite located at 107 Railroad Ave, Morrill, NE:  The employer had two fork trucks, a Nissan and a Datsun, which have illegible identification plates. 29 CFR 1910.178(o)(2):     Load(s) were being handled which exceeded the rated capacity of the industrial truck:      Worksite located at 107 Railroad Ave, Morrill, NE:  The employer failed to ensure that powered industrial trucks were used within their rated capacities.  On February 10, 2015, an employee attempted to lift a pallet which exceeded the capacity of the lift by 1,550 pounds based upon the height of the load.  The forklift in question has a total lift capacity of 2,000 total pounds (based upon the load chart on the lift which is somewhat illegible) and the pallet in question weighed 2,750 pounds.  The fork truck pitched forward until the load struck the ground raising the rear wheels from the ground causing the employee to strike his head on the roll over protective structure.         Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

29 CFR 1910.272(j)(2)(ii):     The employer did not immediately remove any fugitive grain dust accumulations whenever the exceeded 8-Jan inch (.32cm) at priority housekeeping areas, pursuant to the housekeeping program for grain elevators:    Work site located at 107 Railroad Ave, Morrill, NE; throughout the facility:  The employer has failed to ensure that fugitive grain dust was kept below the depth of 1/8 inch in priority areas.  In the upper areas of the mill/storage area, within 35 feet of an inside bucket elevator, the depths ranged from 1/8th inch to as deep as 2.25 inches.  This included the floors and wall ledges.        Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.
