29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a safety monitoring system.    a). Well House - On or about March 5, 2015, employees were working on the roof performing roofing activities that included installation of hurricane roof straps, fascia board, and decking without any fall protection system in place, thus exposing employees to a fall hazard of 7.11 feet to earthen ground.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  a). Well House - On or about March 5, 2015, the employer did not have an adequate fall protection training program that addressed fall protection requirements for employees conducting roofing activities that included installation of hurricane roof straps, fascia board, and decking, thus exposing employees to a fall hazard of 7.11 feet to earthen ground.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a). Eastside of Well House - On or about March 5, 2015, the ladder section used to gain access to the roof did not extend 3 feet above the roof landing surface, thus exposing employees to fall hazard of 7.11 feet onto earthen ground.
