29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing and labeling: On or about March 3, 2015, at Trinidad Rig 138, listed or labeled equipment was not installed and used in accordance with manufacturers instructions exposing employees to electrical shock hazards in the following instances; a. Behind the toolpusher, bunk and living quarter trailers; during wet conditions, five clamp lights (heat lamps) were used as a source of heat to keep the water faucets from freezing up. Manufacturer instructions specifically state do not use in wet conditions. b. Behind the toolpusher, bunk and living quarter trailers; three extension cords were plugged into one another (daisy-chained). Manufacturer instructions specifically states do not plug one extension cord into another. c. Behind the toolpusher trailer; a 6-foot black (for indoor use only)extension cord supplying power to the heat lamp used as a source of heat to keep the water faucet from freezing up. Manufacturer instructions specifically state for indoor use only.
