29 CFR 1926.21(b)(2): Employees were not protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations by placing and keeping such materials or equipment at least 2 feet (.61 m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary:       Instances including, but not limited to the following:     a) Jobsite, 174 Iron Run Road, Bethel Park, PA: On or about March 5, 2015, during excavation activities, the employer did not ensure that previously removed materials were properly maintained by an excavation at the rear of the property in that they were measured approximately 12 to 14 inches from the edge of the top of the excavation and no retaining devices were used.       MWA Acquisitions LLC DBA Matthews Wall Anchor Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.651(c)(2), which was contained in OSHA inspection number 915482, citation number 1, item number 1a, and was affirmed as a final order on August 2, 2013, with respect to a workplace located at 1429 Deepwood Drive, Pittsburgh, PA.

29 CFR 1926.652(a)(1): Each employee in an excavation was protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c):       Instances including but not limited to the following:     a) Jobsite, 174 Iron Run Road, Bethel Park, PA: On or about March 5, 2015, during excavation activities, the employer did not ensure that appropriate cave in protection was in place in the excavation at the rear of the garage area. The excavation was measured to range from approximately 6 feet, 3 inches to 8 feet in depth, with vertical/near vertical excavation walls and undercut wall areas, exposing an employee to a wall collapse.         MWA Acquisitions LLC DBA Matthews Wall Anchor Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 915482, citation number 2, item number 2, and was affirmed as a final order on August 2, 2013, with respect to a workplace located at 1429 Deepwood Drive, Pittsburgh, PA.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees:   Instances including but not limited to the following:   a) Jobsite, 174 Iron Run Road, Bethel Park, PA: On or about March 5, 2015, during excavation activities, the employer did not ensure that a safe way in/out of an excavation at the rear of the property was provided. An employee was transversing an approximate 40 - 48 inch elevation difference manually to get in/out of the excavation that was measured at up to approximately 6 feet deep.
