29 CFR 1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage: On or about March 3, 2015, at the mud tanks of Bison Rig #14, the 120V-nominal electrical cable connected to the external flood light above the tanks had insulation damage leaving the conductors inside exposed to further damage and exposing employees to electrical shock hazards.
