29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not using electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed: On or about March 5, 2015, the employer failed to ensure that employees were protected from electrical hazards in the workplace. An employee suffered an electrical shock and injury while changing a metal-halide light bulb of 270 volts that was suspended from the ceiling 11 feet above the ground. The employee was not wearing insulated gloves to protect themselves from electrical shock. The lights in the area were on at the time of the incident. The incident occurred on or about the morning of February 26, 2015.
