29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard: On or about March 04, 2015 and at times prior thereto, employees were working on Rig #57. One of the suitcases near the mud pumps had the cover bent up to approximately 2.5 inches. This condition created a trip hazard for employees.

29 CFR 1910.23(a)(8)(ii): Every floor hole into which persons could accidently walk was not guarded by a floor hole cover of standard strength and construction: On or about March 04, 2015 and at times prior thereto, employees were working on Rig #57. On the metal suitcases near the mud pumps, there was an open hole which measured approximately 36 inches by 16 inches. This condition created a fall and trip hazard for employees.

In the top draw house, the Camp transformer No. 2 next to air conditioner No. 1 was against the wall inhibiting its ventilation. This exposed employees to a fire hazard.
