29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Drill site: on or about March 4, 2015 and at times prior thereto, a) the metal suitcase at the derrick's base (substructure) was sticking up approximately 2-inches; b) the metal suitcase behind the derrick's base was sticking up approximately 3-inches. these conditions exposed the employees to trip and fall hazards.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Drill site: On or about March 4, 2015, and at times prior thereto, the work platform at the end of the mud tanks (pits) was not guarded. This condition exposed the employees to fall hazards greater than four feet.
