29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling.  On or about 6 March 2015, and at times prior thereto, Re-locatable Power Taps (RPTs) were connected in series to each other from a wall outlet within the administrative offices in a manner not in accordance with the manufacturer's instructions, exposing employees working in the building to a fire hazard. Instance A) Within the Pricing Analyst Office, two RPTs were connected in series with one connected directly to a wall outlet. A computer, monitor, and speakers were plugged in to the last RPT at the time of the inspection. Instance B) Within the Print Shop Office, two RPTs were connected in series with one connected directly to a wall outlet. Various electrical items were plugged in to both RPTs at the time of the inspection.

29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910.  On or about 6 March 2015, and at times prior thereto, an electrical breaker panel located within a storage room off of the employee break room was blocked with materials stored in front, restricting quick and ready access, and exposing employees needing to access the breakers during an emergency to a tripping hazard. Also, some of the items in front of the panel consisted of flammable materials, exposing employees in the area to a fire hazard. The panel was at approximately 120 volts.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy.  On or about 6 March 2015, and at times prior thereto, two finished wall outlets were missing the faceplates within the administrative area of the facility, exposing the employees using them to an electrical hazard of approximately 120 volts. Instance A) Within the Human Resource Manager's Office, a two-plug wall outlet was missing the faceplate and had both plugs being used at the time of the inspection. Instance B) Within the Receptionist Area, a two-plug wall outlet was missing the faceplate and had one plug being used at the time of the inspection.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws.  On or about 6 March 2015, and at times prior thereto, a flexible cord and cable utilized within an administrative office to provide power for a printer was damaged at the cord end, exposing employees to an electrical hazard of approximately 120 volts.
