29 CFR 1904.29(b)(3): Each recordable injury or illness was not recorded on the OSHA 300 Log and 301 Incident Report within seven calendar days of receiving information that a recordable injury or illness has occurred:    a.      On March 02, 2015, in a hotel room 627, a housekeeper incurred acute respiratory symptoms after inhaling a breath from a cloud of dust or powder of unknown origin while cleaning the hotel room.  The employer did not complete nor maintain an entry on its OSHA 300 Log and a 301 incident report on this recordable injury.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:   a.      On or about March 06, 2015, employer allowed employees to use dust masks on a voluntary basis while working in the hotel and the employer had not provided those employees with the information contained in Appendix D of 29 CFR 1910.134 respiratory protection.
