29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of carbon monoxide listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 50 ppm:     (a) On or about March 3, 2015, at Kellys Tavern, a restaurant employee involved in dishwashing duties at the back of the kitchen was exposed to carbon monoxide at a time weighted average (TWA) of 323 parts per million (ppm), approximately 6.5 times the OSHA Permissible Exposure Limit (PEL) of 50 ppm.        (b) On or about March 3, 2015, at Kellys Tavern, a restaurant employee involved in food preparation duties at the back of the kitchen was exposed to carbon monoxide at a TWA of 263 ppm, approximately 5.3 times the OSHA PEL of 50 ppm.       (c) On or about March 3, 2015, at Kellys Tavern, a restaurant employee involved in food preparation duties at the back of the kitchen was exposed to carbon monoxide at a TWA of 256 ppm, approximately 5.1 times OSHA PEL of 50 ppm.         (d) On or about March 3, 2015, at Kellys Tavern, a restaurant employee involved in stacking and food preparation duties at the back of the kitchen was exposed to carbon monoxide at a TWA of 258 ppm, approximately 5.2 times the OSHA PEL of 50 ppm.         (e) On or about March 3, 2015, at Kellys Tavern, a restaurant employee involved in kitchen supervision duties at the back of the kitchen was exposed to carbon monoxide at a TWA of 243 ppm, approximately 4.9 times the OSHA PEL of 50 ppm.
