(f)   Facility:  On or about March 19, 2015, the operator(s) of the Pacific 400 ton hydraulic press brake was/were not protected from having any part of his body in the danger zone when the press is operated.         (g)  Facility:  On or about March 19, 2015, the #18 Bertsch pyramid roll was not equipped with an electrically interlocked cable or kick plate which ran the length of the rollers.     (h)  Facility:  On or about March 19, 2015, the operator of the Roundo R-3 angle roller was not protected from having any part of his body in the danger zone when the angle roller is activated.     (i)   Facility:  On or about March 19, 2015, the operator(s) of the Chicago 750 ton hydraulic press brake was/were not protected from having any part of his body in the danger zone when the press is operated.     (j)  Facility:  On or about March 15, 2015, the operator(s) of the 16 foot pyramid (World War I surplus) was not equipped with an electrically interlocked cable nor kick plate which ran the length of the rollers.     (k)  Facility:  On or about March 15, 2015,  the guard on the Hoan 10 foot shear had an opening of 1/2 inch.  Employees could be injured by either the hold downs which were directly behind the guard or by the shear itself which was about 1 1/2 - 2 inches from the shear point.  This condition presents a crushing or amputation hazard to employees.
