29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about March 2, 2015, the employer did not ensure that employees utilized energy control procedures to control hazardous energy prior to performing allergen cleans on the vertical dough conveyor. Employees were exposed to caught in hazards such as bone fractures and amputations in that the vertical dough conveyor remained in operation as employees removed excess dough from the conveyor, rollers, and roll scrapers.

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) On or about March 2, 2015, the employer did not ensure that the vertical dough conveyor was shut down and de-energized prior to engaging in allergen clean activities. Employees performing allergen cleans did not de-energize the vertical dough conveyor prior to starting work. Employees were exposed to caught in hazards such as but not limited to bone fractures and amputations as conveyor rollers remained energized as employees removed excess dough from the conveyor, rollers, and roll scrapers.
