OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees.  a) On or about March 9, 2015 or prior thereto, in the back automotive bay, employees were exposed to struck-by hazards while working around automotive lifts including a 9,000 Pound Capacity Challenger Lift, Model 14341, Serial Number 4326.  Employees were working underneath raised vehicles and the lift had not been inspected, nor had employees been trained in its use.  b) On or about March 9, 2015 or prior thereto, in the back automotive bay, employees were exposed to struck-by hazards while working around automotive lifts including a 9,000 Pound Capacity Challenger Lift, Model 14341, Serial Number 4327.  Employees were working underneath raised vehicles and the lift had not been inspected, nor had employees been trained in its use.  c) On or about March 9, 2015 or prior thereto, in the front service bay, employees were exposed to struck-by hazards while working around automotive lifts including a 3,000 Pound Capacity Lift, Model and Serial Number unknown.  Employees were working underneath raised vehicles and the lift had not been inspected, nor had employees been trained in its use.  d) On or about March 9, 2015 or prior thereto, in the middle automotive bay, employees were exposed to struck-by hazards while working around automotive lifts including a Rotary Lift, Serial Number 050H1000M-JA.   Employees were working underneath raised vehicles and the lift had not been inspected, nor had employees been trained in its use.  Abatement certification and documentation is required for this item.

29 CFR 1910.215(a)(4):   On offhand grinding machines, work rests were not used to support the work. Work rests were not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage.   a) On or about March 9, 2015, or prior thereto, in the back automotive bay, a 6" Abrasive Wheel Grinder used for machining bolts was missing a work rest.  The external guard was 0.375" from the abrasive wheel.  Abatement certification is required for this item.

Inspections for constant or intermittent jack use at one locality, did not occur at least once every 6 months.  a) On or about March 9, 2015 or prior thereto, in the back automotive bay, a yellow transmission jack had a worn label that partially indicated a 1,100 pound capacity.  It had not been inspected.    b) On or about March 9, 2015 or prior thereto:  An orange transmission jack was missing a label indicating its capacity.  It had not been inspected.
