29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  a)  Fabrication Shop - On or about March 11, 2015, the employer did not develop specific hazardous energy control procedures for the Accurpress 175T Press Brake (Model #717512).  b)  Fabrication Shop - On or about March 11, 2015, the employer did not develop specific hazardous energy control procedures for the Wysong 90T Press Brake (Model #H-90-6).  a)  Fabrication Shop - On or about March 11, 2015, the employer did not develop specific hazardous energy control procedures for the Strippit Mechanical 30T Punch Press (Model Super 30-30).

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  a) Throughout Facility - On or about March 11, 2015, employees who perform servicing and maintenance, to include set up such as die changes, were not trained to the level required for authorized employees in employees were not trained in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  b)  Throughout Facility - On or about March 11, 2015, employees who operate equipment on which servicing and maintenance are performed, were not trained to the level for affected employees, in that each affected employee was not instructed in the purpose and use of the energy control procedures.  c)  Throughout Facility - On or about March 11, 2015, all other employees whose work operations are or may be in an area where energy control procedures may be utilized, were instructed about the procedure, and about the prohibition relating to attempts to restart or reenergize machines or equipment which are locked out or tagged out.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):     (a)  Weld Shop - On or about March 11, 2015, employees operating the Accurpress 175T Press Brake were not protected from having their hands in the danger zone during operation.     (b)  Weld Shop - On or about March 11, 2015, employees operating the Wysong 90T Press Brake were not protected from having their hands in the danger zone during operation.     (c)  Weld Stock Shop - On or about March 11, 2015, no less than two points of operation of the Geka Hdracrop 55/SD Ironworker were not guarded in that the guards were missing on the discharge side of the Flat Shear and Angle Cutter.
