29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: Food preparing area, On or about March 10, 2015, and at times prior thereto, employees were exposed to a caught-in and/or struck-by hazard while placing and removing food products on a meat tenderizer machine that was not guarded at point of operation.

29 CFR 1910.305(j)(2)(v): A receptacle installed outdoors in a location protected from the weather or in other damp locations did not have an enclosure for the receptacle that was weatherproof when the receptacle was covered (attachment plug cap not inserted and receptacle covers closed): Food Preparing Area, On or about March 10, 2015, and at times prior thereto, employees were exposed to a fire hazard from an 110volt/20 amp receptacle installed in a damp location, while not having a weather proof cover installed.
