29 CFR 1926.451(b)(3): Where guardrails were not installed along the front edge of the platform or all employees were not protected from falling with the use of personal fall arrest systems, the front edge of the scaffold platform was more than 14 inches (a) Vicko Construction, Inc., 10618 Worchester St., Commerce City, CO 80022: On and before March 6, 2015, the employer did not ensure employees working on a scaffold were protected from falls in that an employee was working on a scaffold, more than 14 inches from the face of the work, and was not wearing fall protection. This condition exposed employees to fall hazards of approximately 15 feet.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: (a) Vicko Construction, Inc., 10618 Worchester St., Commerce City, CO 80022: On and before March 6, 2015, the employer did not ensure employees working on a scaffold were protected from falls via the use of guardrails along all open sides and ends of the scaffold. This condition exposed employees to fall hazards of approximately 15 feet. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
