29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition:    Along the manufacturing shop floor, on or about March 10, 2015, and at times prior thereto, employees were exposed to explosion, deflagration and/or fire hazards as a result of the accumulation of combustible dust on horizontal and vertical surfaces such as, but limited to wood, saw dust and cardboard material along various sections of unguarded electrical wiring and equipment parts.    The following areas were identified to have combustible dust material:     a)  Along the east wall at 2 live electrical outlets located approximately 4 feet above the ground level along the metal beams of the wall; where dust accumulation was about 1/8 inch to 6 inches. Both electrical outlets had miter saws connected to them.    b)  Along the north wall above the main electrical breaker enclosure that did not have a dead front and missing knockout seals which had wiring running that was powering equipment tools throughout the manufacturing shop.    c)  At 4 locations along the west wall along the electrical outlets located approximately 8 feet high above the ground level.
