Employees were climbing inside cement trucks for cleaning purposes without using a proper confined space entry program.

29 CFR 1910.303(g)(1)(iii): The employer did not ensure that at least one entrance of sufficient area was provided to give access to the working space about electric equipment 600 Volts, nominal, or less. Employees working in the yard did not have direct access to the electrical panel that supplied power to cement loading equipment. This condition exposed employees to potential fire hazards.

29 CFR 1910.1200(a)(1): The employer did not effectively ensure that the information concerning the hazards of all chemicals to which the employees may be exposed to, was transmitted as accomplished by the means of the employer's comprehensive hazard communication programs including labeling and other forms of warning, material safety data sheets and employee training. Employer did not ensure chemical tank located thought out the facility were labeled accordingly with the Globally Harmonized System of Classification.
