OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that an employee was exposed to a fall hazard: a) On or about March 10, 2015, an employee was exposed to a 12 foot fall hazard, in that, an articulating boom forklift was used to elevate the employee on an unsecured wooden pallet to perform steel erection work.

At the site, the operator of an articulating boom fork lift raised an employee to a height of 20 feet on a wooden pallet, exposing him to a fall, on or about 03/10/2014.
