29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  (a) Dan ONeill dba ONeill Roofing on or about March 09, 2015, at the site of Lot 356 Wyllys Dr. North Ridgeville, OH: The employer did not have a plan or program to identify job site hazards for employees to maintain a safe working environment.  Such a program would allow the employer to identify potential hazards such as falls and struck-by during job site operations, decreasing the chance of employee injury and/or illness.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  Dan ONeill dba ONeill Roofing on or about March 09, 2015, at the site of Lot 356 Wyllys Dr. North Ridgeville, OH: The employer failed to ensure employees were protected from falls while installing new roofing material to a residential structure.  Employees working in this condition were exposed up to a 18 ft. fall to the ground below.
