29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):   a. 80 Doremus Avenue, Newark, NJ - Yard and Tank Wash Facility  Employees we not provided training and did not receive certification to operate Kalmar Industries, Model 4X2, yard dogs (Yard Dogs, Spotter Trucks, Yard Jockey, Mule), to include, but not limited to Serial number 31660,  to move ISO tanks into and out of the tank wash facility.  Violation occurred on or about May 9, 2015
