29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition. a) On or about March 9, 2015 at the receiving bump door threshold, the concrete had broken away from the threshold creating a hole in the concrete. Employees were exposed to a fall hazard causing cuts, scrapes, and broken bones.
