29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: Employees were exposed to an electrical shock injury. In the hall between mold prep and closed mold, between the restrooms and providing 110 VAC power to the drinking fountain, the outlet box was not provided with a cover exposing employees to the energized conductors of the outlet.
