This violation occurred at the well site off of County Road 2401 exposing employees to fire and explosion hazards.

29 CFR 1910.106(b)(6): In locations where flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition.  Sources of ignition may include open flames, lightning, smoking, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat.    On or about March 10, 2015 the employer did not ensure that precautions were made to prevent sources of ignition in areas where flammable vapors were present exposing employees to fire and explosion hazards in the following instances:    1) Allowing a pickup truck to come into close vicinity to the well head    2) Allowing smoking near the well head    Mason Well Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.106(b)(6), which was contained in OSHA inspection number 958669,  citation number 1, item number 1 and was affirmed as a final order on April 17, 2014, with respect to a workplace located at Sherrod Unit 2421 outside of Midkiff, TX 79755.
