29 CFR 1904.7(b)(1)(iv): The employer did not record a work-related injury that resulted in medical treatment beyond first aid: a) On or about March 6, 2015, A work related injury which resulted in medical treatment beyond first aid was not recorded on the 2015 OSHA 300 Injury and Illness log. An employee suffered a crushed left pointer finger on or about January 8, 2015, which he received first aid only. The employer continued to work in the shop, and during this period infected his finger. On or about February 11, 2015, the employee returned to Med Express and was diagnosed with an infection in addition prescribed an antibiotic- Bactrim DS 800-160 mg tablets.
