29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. At the job site, on March 4, 2015, the employer did not ensure an employee, who climbed the bracing of the center interior wall frame to access the top of the wall where he performed working tying two frame walls together, wore fall protection, exposing the employee to the hazards associated with death, broken bones, sprains, lacerations, and bruises.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: a) At the job site, on or about and at times prior to March 4, 2015, the employer did not ensure two flexible extension cords used to supply power to a variety of hand tools were equipped with strain relief that was continuous and undamaged.
