29 CFR 1910.107(b)(5)(i): Visible gauges or audible alarm or pressure activated devices were not installed to indicate or insure that the required air velocity is maintained. Filter rolls were not inspected to insure proper replacement of filter media:    (a) At the spray booths, on or about 3/10/2015: The spray booths had no functioning pressure gauges and the filters were in poor condition, damaged and with gaps allowing air to pass through unfiltered.      Abatement certification must be submitted.

29 CFR 1910.134(c)(1): 29 CFR 1910.134(c)(1): In any workplace where respirators are necessary to protect the health of the employee or whenever respirators are required by the employer, the employer shall establish and implement a written respiratory protection program with worksite-specific procedures. The program shall be updated as necessary to reflect those changes in workplace conditions that affect respirator use. The employer shall include in the program the following provisions of this section, as applicable:  *Procedures for selecting respirators for use in the workplace;  *Medical evaluations of employees required to use respirators;  *Fit testing procedures for tight-fitting respirators;  *Procedures for proper use of respirators in routine and reasonably foreseeable emergency situations;  *Procedures and schedules for cleaning, disinfecting, storing, inspecting, repairing, discarding, and otherwise  maintaining respirators;   *Procedures to ensure adequate air quality, quantity, and flow of breathing air for atmosphere-supplying respirators;  *Training of employees in the respiratory hazards to which they are potentially exposed during routine and emergency situations;  *Training of employees in the proper use of respirators, including putting on and removing them, any limitations on their use; and   *Procedures for regularly evaluating the effectiveness of the program.    (a) At the facility, on or about 3/10/2015: The employees were required to use respirators for spray operations including but not limited to North half face cartridge respirators and no respirator program was in effect, there had been no medical evaluations, fit testing or other required respirator program elements.    Abatement certification must be submitted.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    (a) At the work stations, on or about 3/10/2015: The employees sprayed chemicals including but not limited to toxic eye irritant Dupont 858G-210Topcoat, and the plumbed eyewash was not functioning.     Abatement certification must be submitted.
