29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a)Frias Drywall at 3141 Walnut Street, Denver, CO 80205: On and before March 6th, 2015, the employer did not make sure each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Employees worked on a 4-story commercial building where the ladder access/egress, and several window openings were not adequately guarded on each floor. This condition exposed employees to fall hazards up to 52 feet.

Employees worked on a 4-story commercial building where the ladder ingress/egress, the elevator shaft, and several window openings were not properly guarded on each floor. This condition exposed employees to fall hazards up to 52 feet.
