29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:     While working on the Pro-Eco #3 Slitter Machine, employees were not trained to apply energy control devices while entering through the perimeter safety guards into the point of operation.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) covered the following elements, however, the actions were not done in sequence as required by 29 CFR 1910.147 (d)(1)-(6):       The established energy control procedures were not applied when machine operators performed servicing and maintenance tasks at machines including, but not limited to the Pro-Eco #3 Slitter Machine, while exposed to energized and unguarded parts.  The employer did not ensure:         a.    Authorized employees had knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy.   b.    Equipment was turned off or shut down using established procedures.   c.    All energy isolating devices that were needed to control the energy to the machines were physically located and operated in such a manner as to isolate the machinery from the energy sources.      d.    Lockout devices were affixed to each energy isolating devices by authorized employees.
