Employees failed to utilize the Wegmans Lockout Tagout Procedure, Spiral Freezer 3 ( Rev Date Nov. 2014) to lock out the spiral conveyor prior to performing this cleaning operation exposing them to caught between hazards.      b) On or about 03/23/15 in the Bakeshop; Mechanics involved in the removal of a steam reducer valve controlled the low pressure steam (12 psi ) by closing the isolation valve supplying the reducer and hanging a tag on the handle.  Employer failed to develop and document the use of a lockout procedure while performing work on a steam line exposing employees to contact with steam and thermal burn.            ABATEMENT DOCUMENTATION REQUIRED    Wegman's Food Markets, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147 (c) (4) (i), which was contained in OSHA inspection number 315500165, Citation Number 2, Item Number 4a, Repeat, and was affirmed as a final order on 8/8/12, with respect to a workplace located at 1500 Brooks Avenue, Rochester, New York, 14624.
