29 CFR 1910.138(a): General requirements. The Employer did not select and require employees to use appropriate hand protection when employees' hands are exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes. Employees from Bohn and Dawson, Incorporated were not protected by gloves where there was a possibility of thermal burns from skin contact with liquid propane during fuel tank changes.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard were being followed. Bohn and Dawson, Incorporated did not conduct periodic inspections. The company had not reviewed the procedures or conducted periodic inspections on the authorized employees for various machines to include several punch presses, tubing benders, CNC machines, horizontal lathes, notchers, and milling machines.

29 CFR 1910.178(p)(1): A powered industrial truck was found to be in need of repair, defective, or in any way unsafe, the truck and was not be taken out of service until it had been restored to safe operating condition. 3/10/2015, employees were operating unsafe powered industrial trucks, which were in need of repair and not removed from service. The following forklifts were on site and not removed from service: a) Caterpillar forklift, serial # FL1886, was missing an indexing/locating pin and the tank was improperly installed. b) Tusk forklift, serial # FL1882, was missing an indexing/locating pin and the tank was improperly installed. The forklift was being operated during the inspection. c) Komatsu forklift, serial #FL1890 was missing an indexing/locating pin. d) Caterpillar forklift, located in the loading dock area, was missing an indexing/locating pin.

29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service. Such examination was not made at least daily. Forklift operators at Bohn and Dawson, Incorporated had not examined powered industrial trucks daily. Four forklifts needed maintenance and were not removed from service. The forklifts were missing locator/indexing pins. Two forklifts had an improperly installed LPG tanks. None of the forklifts had defects identified nor had the forklifts been removed from service.
