29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) Facility, 166 Dundaff Street, Carbondale, PA - The employer did not develop specific energy control procedures for employees who performed servicing and maintenance on equipment including but not limited to hydraulic presses and radial arm saws, exposing employees to the release of uncontrolled hazardous energy, on or about March 13, 2015.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a) Shear Room, 166 Dundaff Street, Carbondale, PA - Employees processed wire with the RIMA (QPI ID No. 214), a 300kN Maypres Hydraulic Press Model MKN1 30/5 (Serial No. 3135/2020), without a guard to protect the operator's hands and fingers from the point of operation, exposing employees to crushing and caught-in hazards, as observed on or about March 13, 2015.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
