OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to the hazards of broken bones, crushing, and death, from being struck by a Yale, model 1700, front end loader and a Trojan, Model 1700, front end loader operating in reverse with defective/non-operational back-up/reverse alarms and defective/non-operational horns.     Location: Recycling Yard, 36 Brewery Street, Waterbury, CT 06708    Among other methods, feasible and acceptable abatement methods include, but are not limited to, 1) Ensure that back-up/reverse signal alarms are operating, 2) Train or retrain operators and other potentially exposed personnel about the hazards of working in the proximity of the front end loaders, 3) Implement an inspection checklist (to include testing of the back-up/reverse alarms and horns) for each work shift for the front end loaders and any and all equipment that operates in the reverse direction and train the maintenance personnel and equipment operators on the requirements of the inspection procedures. 4) Post safety rules at the entry to yard and establish safe speed limits and train all equipment operators in the safety rules and speed limit in the yard.
