29 CFR 1910.146(c)(4): The written permit space entry program was not available for inspection by employees and their authorized representatives:  On or about March 16, 2015, at the above addressed jobsite, an employee entered the center fuel cell of a 767-300 aircraft without having a confined space permit on site issued by Floats & Fuels Cell identifying the entrant, attendant and supervisor or of the atmospheric hazardous conditions.

29 CFR 1910.146(f)(11): The entry permit that documented compliance and authorized entry to a permit space did not identify the rescue and emergency services to be summoned and the means for summoning those services.  On or about March 16, 2015, at the above addressed jobsite, the job site confined space permit did not address the rescue and emergency services protocol for the airport operations in the event of an emergency.

29 CFR 1910.146(d)(3)(iv): Purging, inserting, flushing, or ventilating the permit space as necessary was not performed to eliminate or control atmospheric hazards;  On or about March 16, 2015, at the above addressed jobsite, an employee was exposed to an asphyxiation hazard when entered the center fuel cell of a 767-300 aircraft without the employer ensuring proper ventilation/purging of all the fuel cells chambers.

29 CFR 1910.146(i)(5): The employer did not ensure that each attendant communicated with authorized entrants as necessary to monitor entrant status and to alert entrants of the need to evacuate the space as required under.   On or about March 16, 2015, at the above addressed jobsite, an employee entered the center fuel tank of a 767 -300 aircraft and the employer did not ensure the attendant was able to communicate with the authorized entrant while in the aircraft confined space as necessary to monitor entrant status and to alert the entrant of the need to evacuate the space.
