K & S Masonry LLC worksite located at 12710 Roachton Rd., Perrysburg, Ohio: On or about March 11, 2015, the employer did not ensure the competent person on the site conducted frequent and regular inspections to ensure employees were protected from hazards including but not limited to; fall hazards and ladder use.

29 CFR 1926.451(e)(1): Scaffold platforms more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used.       A. K & S Masonry LLC worksite located at 12710 Roachton Rd., Perrysburg, Ohio: On or about March 11, 2015, the employer did not ensure a safe means of access was provided for employees accessing the Hydro-Mobile M1 scaffold. The exposed employees were climbing the mast tower to access the platform.

29 CFR 1926.501(b)(1) Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.      A. K & S Masonry LLC worksite located at 12710 Roachton Rd., Perrysburg, Ohio: On or about March 11, 2015, the employer did not ensure employees were protected from fall hazards while performing masonry related activities on the multi-story commercial structure. The exposed employees were landing material on the northwest corner of the structure exposed to a fall distance of approximately 24.00 feet.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
