29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances, severe cuts or lacerations, severe abrasions, punctures, chemical burns, thermal burns, and harmful temperature extremes.  On or about March 11, 2015 at the above addressed jobsite, employees were dispensing LP gas with no hand protection, exposing the employees to a fire hazard.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  On or about March 9, 2015, the employer reported a hospitalization for an accident that had occurred on March 2, 2015.
