29 CFR 1910.147(c)(7): The employer shall provide training to ensure that the purpose and functions of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees.  The training shall include:  Each authorized employees performing maintenance and preventive maintenance on hard wired equipment such as the 24H G&E Gear Hobber, Fellows Type 7 Gear Shaper, Gisholt #5 Turret lathe and etc., were exposed to electric shock / struck by / caught in between hazards due to no training in the recognition of the applicable hazardous energy sources or the methods and means necessary for energy isolation and controls.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  Employees working in shop B were exposed to struck by hazards in which a pedestal grinder operating at 3,450 revolutions per minute had an unadjusted work rest in the following instances.   a) Right side abrasive grinding wheel was approximately 1-1/2 inches from the work rest.   b) Left side abrasive grinding wheel was approximately 3/4 of an inch from the work rest.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  Employees working on shop B were expose to struck by hazard in which a pedestal grinder operating at 3,450 revolutions per  minute had an unadjusted tongue guard in the following instances:  a) Right abrasive grinding wheel was approximately 1-3/4 inches from the tongue guard.   b) Left abrasive grinding wheel was approximately 1 inch from the tongue guard.

29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting:   Employees operating the Key seat cutter (Lincoln TV 2996) in shop B were exposed to struck-by hazards due to an unguarded rotating shaft.  The shaft was approximately 6-1/2 inches long, 3 inches in diameter, and was rotating at approximately 1,745 revolutions per minute.

29 CFR 1910.219(d)(1): Guarding. Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes:   Employees working on the following equipment were exposed to caught in between hazards from unguarded pulleys that wear less than 6 ft from the floor in the following instances:   a) OD Grinder, model SC-224- JE 2-1 had an unguarded pulley opening that was 5 inches high and 13 inches long.  The unguarded pulley was approximately 5 feet from the floor and was operating between 2,880 and 3,450 revolutions per minute.      b) Lathe Gisholt Equipment model# 13W9107A was being operated at 1,750 revolutions per minute in shop C,  there was a 10 by 18 inch section of an unguarded pulley that was behind the machine approximately 3 feet from the floor.

29 CFR 1910.219(e)(1): Horizontal belts which had both runs 42 inches or less from the floor level were not fully enclosed by guards conforming to the requirements specified in 29 CFR1910.219 (m) and (o):  Employees working on shop C were exposed to caught-between hazard from unguarded portions of belts that wear less than 6 feet from the floor in the following instances:  a) OD Grinder, model number SC-224- JE 2-1, operating between 3,450 and 2,880 rpm, had an unguarded 13 inch long rotating belt.   b) Lathe Gisholt model number 13W9107A operating at 1,750 rpm had an unguarded 18 inch long rotating belt on the back side of the machine.
