29 CFR 1910.147(c)(6)(ii): The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.   a.     Recordkeeping - On or about March 12, 2015, employees were required to work on various machines including milling equipment using specific lockout tagout procedures. The employer did not certify periodic inspections were conducted by authorized employees, in accordance with the aforementioned standard; to include the machine or equipment on which the energy control procedure was being inspected, the date of the inspection, the employees included in the inspection, and the person performing the inspection; thereby exposing employees to the hazards associated with unexpected re-energization of equipment.

29 CFR 1910.178(l)(6): Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.   a.     Recordkeeping - On or about March 12, 2015, employees were required to operate powered industrial vehicles on a daily basis. The employer was not able to provide documentation that employees engaged in the operations of powered industrial vehicles had been certified in accordance with OSHA standards.

29 CFR 1910.179(j)(2)(iv): Hoist chains, including end connections, for excessive wear, twist, distorted links interfering with proper function, or stretch beyond manufacturer's recommendations. Visual inspection daily; monthly inspection with a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected.  a.     Recordkeeping - On or about March 12, 2015, Employees in the Shipping / Receiving area were required to work with a 1 ton hoist. The employer failed to document inspections as performed by the operator on a monthly basis, to include signed certification records.
