On or About March 12, 2015 LOCATION; 155 East 79th Street, New York, NY 8th Floor By Stairway A A) Employees working on the 8th floor were exposed to energized parts in a live 220 VAC circuit breaker panel box which was not protected or covered.

29 CFR 1926.502(i)(4): All covers were not color coded or marked with the word "HOLE" or "COVER" to provide warning of the hazard: On or About March 12, 2015 Location: 155 East 79th Street, New York, NY, 11th, 10th, 8th, and 6th floors a). Employees were exposed to trip and fall hazards because the floor holes, approximately 36" x 8", adjacent to the shafts were not covered with covers bearing the words "HOLE" or "COVER".
