29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On or about March 12, 2015 Location 155 East 79th Street, New York, NY 11th floor, Northeast Corner (a) Employee was sweating the joint on 3/4" copper domestic water riser pipes and was exposed to sever eye injuries because he was not wearing suitable eye and face protection to prevent burns from the use of a plumbers MAP GAS torch, and 50-50 Tin-Lead solder.
