29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about March 11, 2015, at the T6 wall form located at 7200 West Road Way, New Orleans, Louisiana the employer did not instruct each employee so that they adequately recognized and avoided unsafe conditions. The 3/4" X 2" quick bolts were removed from both sides of the 3R x 12 panel.
