29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: (a). Worksite located at 1408 Bus. Loop I-70, Columbia, Missouri- Employees were exposed to a shock or electrocution hazard in that cords were observed that had its' outer insulation breached and had inner wires exposed. The employees were also splicing cord that had the ground prong missing.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a). Worksite located 1408 Bus. Loop I-70, Columbia, Missouri- Employees were observed working and walking on top of trusses that were lying on the exterior walls that were to be stood up and secured. Employees were walking on these trusses without the use of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a). Worksite located at 1408 Bus. Loop I-70, Columbia, Missouri- Employees were exposed to a fall hazard in that the ladder being used to access the top of the trusses that were lying on top of the exterior walls was not extended at least 3 feet above the upper landing surface.
