Residential Renovations, Ltd. worksite located at 10667 Fremont Pike, Perrysburg, Ohio: On or about March 12, 2015, the employer failed to ensure an accident prevention program was in place to aid the employer and his employees in the recognition and avoidance of hazards associated with roofing operations (i.e.; fall protection,use of personal protective equipment, ladder use).

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.    A. Residential Renovations, Ltd. worksite located at 10667 Fremont Pike, Perrysburg, Ohio: On or about March 12, 2015, the employer had not trained employees in the appropriate use of fall protection, such as but not limited to, the use of personal fall arrest systems while performing various roofing operations.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.    A. Residential Renovations, Ltd. worksite located at 10667 Fremont Pike, Perrysburg, Ohio: On or about March 12, 2015, the employer did not provide fall protection for the two employees performing work on the metal roof. Employees were exposed to a fall distance of approximately 20.00 feet.

A. Residential Renovations, Ltd. worksite located at 10667 Fremont Pike, Perrysburg, Ohio: On or about March 12, 2015, the employer did not ensure the side rails of the extension ladder was extended at least three feet above the landing surface. Two employees were exposed to falls while using the ladder to access the roof.
