29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. a) Jobsite On or about March 10, 2015, employees installing sheetrock siding and aluminum metal studs were observed using battery powered DeWalt hand drills without the use of safety glasses, exposing employees to eye injuries from ricocheting and flying debris.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated. a) Jobsite - On or about March 10, 2014, employees were allowed to work from an articulating work platform Model: JLG Lift 400S, S/N 0300104349, without adequate fall protection in that employees were observed wearing a harness without lanyard attached and another employee not wearing a complete personal fall arrest system, exposing the employees to a fall hazard up to 19 feet.
